Results of ultrasound guided aspiration cytology of extrathyroidal neck masses of 73 patients were review-ed. included cases were 23 malignant lesions; 16 metastatic tumors 4 lymphomas 3 salivary gland tumors and 50 benign lesions : 24 tuberculous lymphadenites 15 abscess 3 benign lymph node hyperplasias etc. there were one vase of false negative and none of false positive result for malignancy. in two cases of malignancy insufficient cellular material was obtained. In one case of lymphoma and a benigh lymph node hyperplasia it was difficult to distinguish between benignancy and malignancy on cytologic smear. for the malignant lesions sensitivity was 83% specificity was 98% and overall accuracy was 93% There were none who had suffered adverse effect from the procedure. ultrasound guided aspiration cytology seems to be simple accurate and safe diagnostic modality for neck masses especially in condition that malignancy can not be excluded.
