WE analysed chest radiographic findings of 81 serologically confirmed leptospirosis patients fro Chonbuk National university Hospital, Chonju Presbyterian Medical Center and Namwon Province Hospital from September, 1987 to Jul, 1989. The results are as follows: 1.Incidence fo chest radiographic abnormalities was 52%42 of 81 patients) and incidence of pulmonary parenchymal lesions was 38%(31 of 81 patients) 2. The pulmonary lesions occurred 3-7 days after clinical onset in 28 cases(90%) and 1-2 days in 3 cases. 3. The radiographic patterns of pulmonary lesions were multiple small nodular pattern(16.31, 52%), massive confluent consolidation pattern99/31, 29%), and diffuse interstitial pattern(6/31, 19%). 4. The radiographic patterns of pulmonary lesions showed changes in following sequence. Type I,II,IIIand finally resolution or similar change. 5. The radiographic gradings, representing extent of pulmonary lesions were as follows; GradeI(13%),Grade II(19%), Grade III(16%), Grade IV(52%). 6. The pulmonary lesions were distributed bilaterally in 77% and were predominant in right lung(88%). There was very high tendency of peripheral occurance(91%) and distribution of apex, perihilar region and base of lung was 23%, 26% and 61%, respectively. 7. The resolution of pulmonary lesions was complete except for expired cases and usually tookless than 3 days916%), 4-10 days961%) and longer than 11 days(13%) after their appearance. And resolution took 2-4 days in Grade I cases, 3-10 days in Grade II cases, 4-11 days in Grade III cases and 5-15 days is Grade IV cases. Thus the time of resolution was related to the extent of pulmonary lesions. The lesser the pulmonary lesion, the earlier the resolution. 8. Extrapulmonary mainfestation, such as pleural effusion and cardiac enlargement was rare. In conclusion, radiographic manifestation provided useful information about early diagnosis, prognosis evaluation & treatment of leptospirosis.
