We retrospectively reviewed chest radiographs of 89 patients with pulmonary tuberculosis who had undergone pneumonectomy, lobectromy, or segmentectomy. All the 66 patients without postoperative complication showed gradual elevation of air fluid level within the postresectional space over several weeks to months. Sudden elevation of the air fluid level was seen in patients with bleeding or empyema. Dropping of air fluid level more than 2cm in height suggested bronchopleural fistula. Transient contralateral shift of the mediastinum was seen in 30% of the patients without complication in early postoperative period. However, persistent contralateral shift was seen only in patients with complications such as bronchopleural fistula, bleeding and empyema. Soft tissue emphysema was reduced within several days in patients without complication, while it was increased in cases of bronchopleural fistula, bleeding and empyema. The air fluid level, soft tissue emphysema and location of mediastinum were the key factors in detecting complications in follow up period.
