연구배경: 장기간 약물 치료를 필요로 하는 환자에게 사용되는 NSAID의 부작용을 줄이기 위한 노력으로 신속한 용해도와 흡수력을 개선시킨 ${eta}$-cyclodextrin을 기존의 NSAID에 포접시켜서 그에 대한 유효성과 안정성이 보고되었다. 방법: 1992년 10월 1일 이후 한양대학병원 류마티스 센터로 내원한 골관절염 환자중에서 주로 슬관절의 동통만을 호소하는 환자를 대상으로 28명에게 ${eta}$-cyclodextrin-piroxicam$을 투여하였고 20명 에서 piroxicam제제를 투여하고 임상효과 및 부작용을 관찰하였다. 결과: 약물을 투여받은 2군 모두에서 동통, 종창, 관절압통 및 기능 장애 가 평균적으로 개선됨을 관찰할 수 있었으며 ${eta}-cyclodextrin-piroxicam$ 투여군에서 투여를 중단한 경우는 단 1예로 나타났다. 결론: 약물 투여를 장기간 필요로 하는 류마티스 관절염 및 골 관절염에 있어서 ${eta}-cyclodextrin-piroxicam$은 적절한 약제로 사료된다.
Background: The aim of the present study was to evaluate the efficacy and tolerability of ${eta}-cyclodextrin-piroxicam$ in the treatment of osteoarthritis. The cyclodextrins are water-soluble cyclic oligosaccharides produced by enzymic conversion of starch. Drug-cyclodextrin inclusion complexes may increase the solubility of the drug in water, thus enhancing absorption. ${eta}-cyclodextrin$ can form inclusion complexes with drugs such as piroxicam, allowing increased solubility, stability and absorption of the active substance. Method: We studied clinical efficacy and side effects in 20 patients with osteoarthritis receiving piroxicam and in 28 patients receiving ${eta}-cyclodextrin-piroxicam$. It was randomised. controlled study of ${eta}-cyclodextrin-piroxicam$ versus piroxicam. Drug treatment were administered orally, twice daily for 12 weeks. Therapeutic efficacy after 0, 2, 4, 12 week was evaluated by themselves and doctors. Results : It showed the good analgesic effect of piroxicam. There were three drop-out due to gastro-intestinal intolerance in the piroxicam group and one in the ${eta}-cyclodextrin-piroxicam$ group. Conclusion : The study suggests that ${eta}-cyclodextrin-piroxicam$, used in the long-term treatment of rheumatic disease, improves the safety of piroxicam without affecting its efficacy.
