연구목적 치매의 대부분을 차지하고 있는 알쯔하이머병과 혈관성 치매 환자의 인지 기능 장애의 양상에 따른 차이를 알아보고, 인지기능의 효율적인 평가를 위해 전산화된 인지평가도구를 소개하여 이에 대한 활용과 그 가치를 알아보고자 한다. 요약 알쯔하이머병에서는 초기부터 지남력을 포함한 기억력, 이름대기 능력, 그리고 시공간능력이 심하게 손상된다. 반면, 혈관성 치매에서는 주의력, 집행기능, 계획 및 조절능력이 먼저 손상되고 언어의 유창성이 저하되며, 인지 과정의 처리 속도가 느리고 시공간능력 장애와 보속증을 보인다. 이러한 수행양상의 차이를 알아보는것은 치매의 조기진단과 치료를 위하여 의미가 있다. 효율적인 평가를 위한 전산화된 인지평가는 각 인지 기능에 대한 보다 정확하고 세부적인 이해를 증진시킬것이며, 치매 초기의 작업치료적 개입에 중요한 정보를 제공한다. 인지평가도구 중 전산화된 인지평가 참고 진단 시스템( Cognitive Assessment & Reference Diagnosis System, CARDS)은 10개의 섹션으로 구성된 기억력(memory) 검사와 5개의 섹션으로 구성된 기억력이 아닌(non-memory) 검사로 94개의 세부검사로 구성되어 있다. 피검자와의 검사에 평균 30분(20-45분)이 소요되고, 정보제공자와의 면 담에 평균 10분이 소요된다.
Objective : This paper attempts to investigate differences according to an aspect on cognitive dysfunction of people with Alzheimer's disease and Vascular Dementia that take up most of dementia, introduce the computerized cognitive assessment tools to evaluate effectively the cognitive function, and recognize the value and utilization of it. Summary : Alzheimer's disease badly damages memory, ability giving one's name, and temporal and spatial abilities including orientation since early times, while Vascular Dementia damages attention, implementation function, and planning and controlling faculty first, declines fluency in language, slows a processing speed of cognition process, and shows temporal and spatial disorders and perseveration. It is meaningful for an early diagnosis and treatment for dementia to investigate differences of these aspects. The computerized cognitive assessment for an effective assessment will improve more accurate and detailed understanding of each cognitive function and provide important information on the occupatipnal therapeutic intervention of early dementia. The Computerized Cognitive Assessment & Reference Diagnosis System(CARDS) of cognitive assessment tools is composed of 94 detailed tests by memory tests with ten sections and non-memory tests with five sections. It takes 30 minutes (20-45 minutes) to test with subjects and average 10 minutes to interview with an informant.
