당뇨병 환자의 질병체험에서 나타난 당뇨병 관리를 위한 가족관계의 특성

본 연구의 목적은 당뇨병 환자의 당뇨병 관리를 위한 가족관계에 대한 주관적 인식을 분석하고, 가 족관계 및 지지의 특성이 환자의 임상결과와 어떤 연관성을 보일 것인가를 조사하는 것이다. 연구방 법은 이전에 구축된 당뇨병 환자의 질병체험데이타베이스에 포함된 47명 환자의 질병체험 자료를 2차 분석하여, 질적 연구방법으로 당뇨병 환자의 당뇨병 관리를 위한 가족관계의 유형과 특성을 분석하고, 등급평가기준(루브릭)을 이용하여 가족관계 및 지지의 특성과 당뇨병의 효과적 관리와의 상관성을 조 사하였다. 연구결과는 대부분의 환자들이 당뇨병 관리를 위하여 가족관계 및 지지를 중요하게 생각했 으며, 가족지지의 형태는 참여적지지, 부정적 강화 등 다양하게 나타났다. 가족관계 및 지지의 등급의 차이는 합병증 유무에 따른 당뇨병관리의 효과성과는 통계적 유의성이 없었다.

영문초록

Objectives: This study assessed diabetic patients’ perspectives of their family relationships and analyzed the effects of these relationships on clinical outcomes of diabetes management. Methods: Forty-seven (47) diabetic patients’ illness experiences, documented in the Korean Database of Individual Patient's Experience (DIPEx), were analyzed. Types and characteristics of family relationships of these patients were arrived at using grounded theory, and the effects of family relationships on the patients’ clinical outcomes were assessed quantitatively. Results: Most patients recognized that family relationships and family support are important for successful illness management. The types of family support varied, including participatory support and negative reinforcement. The level of family support was not significantly associated with the effectiveness of diabetes management to prevent diabetic complications. Conclusions: Family relationships and support of diabetic patients are meaningful factors in the successful management of diabetes.
