The purpose of this study was to develop health activity program based on the 2015 revised physical education curriculum to help 6th grade students of elementary school form their own exercise habits. To achieve efficiently the result of learning which means exercise habit formation, the health activity program was developed according to the ADDIE model which is a general framework to design instructional courses applying non-exercise physical activity model and habit formation strategies. Seventy-four students(Thirty eight male and thirty six female students) of three 6th grade classes participated in the course of twenty period and the course was evaluated when it was finished. The characteristics of the program including principles of the 2015 revised physical education curriculum were first, it was operated throughout the year to improve their exercise habits, second it was designed for the students to plan exercise schedules and practice constantly by themselves considering their individual physical characteristics. It helped the students form exercise habits, but they had difficulty in writing journals and taking the physical ability test. In conclusion, first, the value of the study is that authentic and constant course was developed focused on the result of the learning for exercise habit formation. Second, the program provides procedural knowledge and operating tips for health activity.
