본 연구는 집단미술치료가 정신요양시설에 입소되어 있는 만성정신분열병 환자들에게 긍정적 자기표현에 미치는 영향을 고찰해 보고자 실시되었다. 연구대상자는 I시 정신요양시설 보호생활관에 입소되어 있는 만성정신분열병 환자 중 자발적 언어 및 행동표현이 가장 결여되어 일상생활 능력 및 운동 기능이 떨어지고 원내 프로그램 참여가 어렵다고 판단되는 환자들로 남성 6명이었으며, 사회복지사에 의해 선별되었다. 연구기간은 2004년 6월 8일부터 동년 12월 21일까지로 주 1회, 총 22회기 실시하였으며 1회 90～120분 소요되었다. 연구방법은 연구자의 지지적 치료접근법으로 미술활동에서 다양한 매체 탐색과 작업과정을 통해 구체적 유형물을 획득하고 자신의 구체물에 대한 실체적 표현 활동을 통해 성취감과 자발적 자기표현이 가능하게 하는 미술치료 프로그램을 시행하였다. 연구측정 도구는 회기별 진행과정에서의 행동변화 관찰 체크리스트와 투사검사로 K-HTP, LMT를 사전, 사후 실시하였다. 본 연구결과, 행동 변화에서는 연구초기에 무기력 상태에서 소극적인 자기표현 행동을 보였으나 회기가 진행됨에 따라 미술활동에 대한 흥미와 성취감을 갖게 되었고 초기와는 다르게 연구자와의 눈 맞춤이나 요구에 의한 상호작용에서 적극적인 자기표현을 나타내었다. 연구초기 연구자의 적극적 인 개입과 지지가 있어야만 작업수행이 가능했던 환자들이 대부분이었으나 연구중반 10회기 이후부 터는 연구자의 적극적인 도움과 지지 없이도 대부분 미술작업 수행이 잘 이루어졌다. 사전, 사후 그 림검사인 K-HTP와 LMT 결과도 사전검사 보다 사후검사에서 대부분 연구대상자들의 그림 구성 통합과 필적이 안정되게 나타났으며, 풍부한 내용과 다양한 색채가 표현되고 형태지각과 입체적 공간 구성력에 향상을 보인 것으로 나타났다. 이와 같은 결과로 만성적 정신병리와 오랜 기간 정신요양시설에서의 생활로 인한 사회적 위축으로 적절한 자기감정 및 행동표현에 어려움을 갖게 되어 대인관계에서 부적절한 상호작용과 잦은 실패감을 느끼며 생활하고 있는 만성정신분열병 환자들에게 미술치료는 활동에 대한 즐거움과 흥미를 유발하였고 수준에 적절한 과제수행에 대한 성취감과 결과물들로 이들의 자기표현에 긍정적 변화가 있었음을 알 수 있다.
The purpose of this study is to see how the self-expression art therapy affects on the chronic schizophrenic patient in a mental health facility. The participants were 6 men, who were in mental health facility of I city, and were diagnosed as chronic schizophrenia, and showed a remarkable rack of spontaneously verbal self-expression and physical self-expression. The total session was 22sessions and It took about 90 minutes to 120minutes per one session. The supportive art therapy was committed to supply a sense of spontaneous expression and achievement for patients while working with a variety of art materials and getting concrete objects. The behavior change observation per sessions were committed and the pre and post test of K-HTP, LMT were committed. Followings were the conclusions from the study results, First, the self-expression art therapy on the chronic schizophrenic patient had a positive effect on patients' spontaneous self-expression, especially the patients showed interest on the art work and made directive eye contact to the researcher comparing the early session. Second, the frequency of spontaneous work without direction or support of co-workers had been increased since 10th session. Third, the result of pre and post test of K-HTP, LMT showed also the stable handwriting and integration of picture constructs. In addition, most pictures were filled with a variety of constructs and colours. Also, the perception of figure and ability of body organization were improved.
