목적 : 본 연구는 국내 노인보건의료 및 복지서비스 대상자의 누락 및 중복, 노인장기요양보험제도의 사각지대, 지역사회 내 공공·민간 복지 서비스 산재 그리고 경도 장애 대상자 제외 등의 문제점을 해결하기 위한 한국형 작업 중심 지역포괄센터 모델의 구축 방안을 제시하고자 실시하였다. 연구방법 : 국내외 노인복지 서비스 관련 법제도 분석, 실태현황 및 문제점 그리고 선진국의 사례를 토대로 한국 실정에 맞는 작업 중심 지역포괄센터 모델을 개발하였다. 모델의 적절성과 국내 적용 가능성 및 효과성을 확인하기 위해 전문가 내용타당도 검증과 포커스 그룹 자문을 통해 최종 모델을 개발하였다. 결과 : 작업 중심 지역포괄센터 모델은 의료연계, 재가급여 강화, 건강증진, 권익증진 및 주거안전 5가지 서비스를 계속적·포괄적으로 제공하는 시스템으로 개발되었다. 내용타당도 검증결과 문항별 타당도 전체 평균은 9점 만점 중 7.16점으로 높게 나타났다. 결론 : 작업 중심 지역포괄센터는 지역사회에 거주하는 노인 및 고령자가 활동 및 참여를 통해 존엄을 유지하면서 가능하면 오랫동안 정든 지역사회에서 지속적으로 생활하기(aging in place) 위해 필요한 의료, 예방, 주거 및 생활 지원 서비스를 제공해 줌으로써 삶의 질 향상에 기여할 것으로 보인다. 또한 작업치료사가 케어매니저 역할을 함으로써 지역사회재활 전문가로 인정받을 뿐만 아니라 이 분야로의 합법적인 진출의 시발점이 될 것으로 기대하는 바이다.
Objective : This study aims at suggesting how to build a Korean model for regional comprehensive center focusing on occupation in order to solve problems such as omitting and overlapping in the subjects for healthcare and social welfare for the elderly in Korea, blank in longterm care for the elderly, scattered services in both public and private sectors in local communities, and exclusion of patients with mild disability. Methods : Based on the analysis on the laws and policies related to social welfare services in Korea and abroad, their current status and problems, and the case studies from the developed countries, a model for regional comprehensive service focusing on occupation, suitable to the Korean circumstances was developed. To confirm the model's appropriateness, its feasibility in Korea, and its effectiveness, contents validity by experts and consulting by focus group interview(FGI) were conducted and then the final model was developed. Results : The regional comprehensive model focusing on occupation was developed as a system to provide continuously and comprehensively five services. As a result of the verification of the content validity, the entire average of validity per question was 7.16 of 9, showing the higher point. Conclusion : Regional comprehensive center focusing on occupation will contribute to enhance quality of life by providing medical, preventative, housing, and living support services required for aging in place in longtime familiar local society as possible, keeping dignity through activities and participations by elderly and aged people in local society. In addition, occupational therapist will play a role of care manager and be recognized as regional social rehabilitation expert, becoming the legitimate beginning to entry into the sector.
