우리나라의 전체 인구는 4,858만명으로 5년 전보다 2.8% 늘었다(Statistics Korea Data [SK], 2010). 이에 비해 노인인 구는 542만 명으로 5년 사이 24%나 증가했고 전체 인구의 11.3%를 차지하였다. 또한, 2009년 평균수명은 남성 77세 여 성 83.8세로 노인인구가 급격히 증가하고 있다(SK, 2010). 수술과 마취 기술의 발달로 인하여 노인 환자에게도 수술 을 권유하는 경우가 많아지면서 수술을 시행 받는 노인 환자 수 도 증가하고 있다. 국민건강보험공단(National Health Insurance Service) 통계자료(2011)에 의하면 2006년 주요수술 에서 65세 이상의 노인이 차지하는 비율이 19.8%에서 2011 년 31.2%로 급격하게 증가하였다. 노인 환자의 수술 위험도 와 수술 결과는 나이, 환자 신체상태 및 동반질환, 수술 종류에 따라 달라진다. 나이와 동반질환은 수술 후 합병증 발생의 중요한 지표로서 수술의 위험성을 높인다(Korean Society of Anesthesiologists, 2009). 노인은 일반 성인과는 달리 다 양한 증상과 여러 질병이 다발적으로 발생하거나 복합적으로 존재하고 있는 경우가 많으며, 노인 환자에게 흔히 나타나는 만성질환은 마취 회복을 어렵게 만든다(Korean Geriatrics Society, 2010). 또한, 수술 후 노인 환자들은 흔히 심혈관계, 호흡기계, 전해질 불균형 등의 문제가 나타날 수 있다(Clayton, 2008).
The purpose of this study was to identify the nursing needs of elderly patients while in the recovery room following spinal anesthesia. Methods: The participants in this study were 120 elderly patients had surgery under spinal anesthesia between from December 12, 2011 and March 31, 2012. The instrument used was a descriptive questionnaire on nursing care needs of patients in the recovery room. For reliability, the instrument had a Cronbach's ⍺ of .882. The data were analyzed using frequencies, percentages, means, t-test, ANOVA, and Cronbach's ⍺ coefficient with the SPSS/WIN 12.0 program. Results: Score for nursing needs of elderly patients with spinal anesthesia who were in the recovery room was 2.47±0.82 of a range 1~5. The scores for individual nursing needs were as follows: educational need (2.99±0.80), physical need (2.62±0.59), spiritual need (2.29±1.12), emotional need (2.08±0.81), and environmental need (1.71±0.83). Conclusion: While taking care of elderly patients in the recovery room after surgery, nurses should provide enough information about the surgery and anesthesia, observe thirst and coldness, and when explaining do it slowly in a loud voice. Spiritual care for elderly patients in recovery room is also important.
