일 년에 한번, 공개채용을 통해 입사한 간호사들은 졸업 후 며칠 이내에 임상현장에 투입되기도 하지만, 몇 개월 혹은 1년 이상을 대기발령인 경우도 있으므로 이른바 다른 의미의 유휴간호사의 역할을 하게 된다. 학생에서 간호사로의 원만한 이행은 간호와 관련된 지식과 업무에의 자신감을 향상시키고(Cockerham, Figueroa-Altmann, Eyster, Ross, & Salamy, 2011) 능력의 통합을 유도하므로(Gregg, Wakisaka, & Hayashi, 2013) 향후 간호사로서 발전해 나가는데 있어서 매우 결정적인 시기라고 할 수 있다. 하지만, 일본 간호협회에 따르면 약 80%에 달하는 신규 졸업 간호사들이 불충분한 지식과 기술 때문에 임상현장에서 1차 스트레스를 받고, 나아가 의료관련 오류 발생에 대한 불안감 때문에 2차 스트레스를 받는 것으로 보고되었다.(Gregget al., 2013). 이는 임상경험이 부족하여 환자 간호에 필요한 고난도의 지식이나 기술이 부족한 반면, 돌봐야 하는 환자들의 중증도가 다양하기 때문에 발생하는 현상으로, 새로 졸업한 간호사들을 위한 적응 훈련 프로그램의 필요성을 보여주는 결과라 볼 수 있다.

영문초록

Purpose: This study was to identify predictors of drug dosage calculation error risk in newly graduated nurses. Methods: A total of 115 newly graduated nurses who passed their employment examination, but didn’t work for hospital yet, were recruited from a university hospital. The data were analyzed by descriptive statistics, X2-test, t-test, ANOVA, Pearson correlation coefficients, and stepwise multiple regression using the SPSS 18.0 program. Results: The mean score of ‘drug dosage calculation ability’ was 0.81±0.16 and the mean score of ‘certainty of calculation’ was 2.95±0.60 out of a 5 point scale. The error risk of drug dosage calculation was positively related to anxiety for drug dosage calculations (r=.388, p<.001), but negatively related to interest and confidence in mathematics (r= -.468, p<.001), confidence related to dosage calculations (r=-.426, p<.001). The main predictors of error risk related drug calculations in newly graduated nurses were identified as interest and confidence in mathematics (β=-.468, p<.001). This factor explained about 21.9% of the variance in error risk of drug dosage calculation. Conclusion: The strategies used to decrease the error risk related drug dosage calculation such as improving interest and confidence in mathematics should be developed and implemented.
