국문초록

한국인의 평균수명은 지난 50년간 20세 정도 증가하여 2012년에 여자는 84.5세, 남자는 77.7세에 이르고 있다(Korea National Statistical Office, 2012). 평균수명의 증가는 노인인구의 연령 별 분포에도 변화를 가져와 60세 이상 인구 가운데 60-74세의 인구가 2005년 77.1%에서 2010년 73.1%로 감소한데 비해, 75세 이상 인구는 22.9%에서 26.9%로 증가하였고 2020년에는 42%까지 증가할 전망이다(Korea National Statistical Office, 2012). 노인인구 가운데에도 후기고령인구의 증가는 노인질환 대부분의 유병율과 노인 의료비를 상승시켜 전 국민의 부담을 가중시킨다(National Health Insurance Corporation [NHIC], 2013). 골관절염은 연령에 따라 유병률에서 큰 차이가 나타나는 대표적인 만성질환이다(American College of Rheumatology [ACR], 2012). 2008년 국민건강영양조사(Ministry of Health and Welfare [MHW], 2008)에서 보고한 본인 인지 골관절염 유병률은 50-59세 인구에서는 15.1%이었으나, 60-69세에는 24.3%, 70세 이상에서는 29.8%로 상승하였다. 또한 2010년 국민건강영양조사(Ministry of Health and Welfare [MHW], 2011)에서 관절 통증과 방사선검사로 판정한 골관절염 유병률은 50-59세 인구에서 5.7%인 데 비해 60-69세 인구에서는 50대의 세 배, 70세 이상 인구에서는 다섯배 증가하였다. 골관절염은 유병률이 높다는 것 외에도, 장애율이 높고(Yoon, 2012), 대상자뿐 아니라 가족의 삶의 질을 저하시키며(Park & Lee, 2012; Yang & An, 2011), 높은 의료이용률에 따른 질병 부담을 증가시킨다는 점에서(Hur, Choi, Uhm, & Bae, 2008) 심각한 건강문제이다. 우리사회가 고령화될수록, 노인 가운데에도 후기 고령자의 분포가 높아질수록 골관절염 인구는 더욱 증가할 전망이다. 우리나라의 골관절염 인구가 노년에 집중되어 있고, 이로 인한 고통과 경제적 손실이 심각하여 적극적인 간호가 요구되는데도 불구하고 국내 골관절염 연구의 대부분은 중년 이후 노년까지 넓은 연령계층을 분석하였기 때문에(Kim & Jeon, 2011; Kim & Kim, 2013; Ko, 2010; Yang et al., 2006), 노인인구의 골관절염을 발생시키는 위험요인에 관해 확인된 바(Yang, 2006)는 제한적이다. 노인 가운데에도 후기 고령인구의 골관절염 특성에 관한 연구는 전무한 실정이다.

영문초록

Purpose: The purpose of this study was to identify the prevalence of osteoarthritis and its risk factors among a community dwelling population age 60-74 and 75 and over (young-old vs. old-old) in Korea. Methods: A total of 1,389 of age 60-74 and 397 of age 75 and over from the 2010 Korean National Health and Nutrition Examination Survey (KNHANES) were selected. The data analysis was performed using the complex sampling function of SPSS version 19.0. Results: The prevalence of osteoarthritis was greater for old-old people (35.50%) compared to young-old people (18.20%). Univariate logistic regression analysis shows that the difference of risk factors for osteoarthritis between young-old and old-old elderly people. The prevalence of osteoarthritis in young-old people was associated with lower education levels; performance of moderate intensity physical activity; higher waist circumference, whereas old-old people was most affected in females; depression and experience. Conclusion: The finding of differences in the prevalence of osteoarthritis and its’ risk factors between young-old and old-old people is expected to promote the screening strategy for the aged at risk of osteoarthritis in the Korean community.
