이 연구는 암환자를 대상으로 한 음악치료가 대상자의 기분상태인 불안, 우울, 분노, 활력에 긍정적인 차이를 보이는가를 규명하는 것이다. 연구대상자는 2008년 3월 25일부터 5월 16일 까지 K대학병원 호스피스·완화병동에 입원한 암환자 10명과 KC병원의 입원환자 10명, 외래 환자 11명으로 모두 31명을 선별하여 실험을 진행하였다. 실험은 주2회씩 4주 동안 총 8회기 의 음악치료를 시행하였으며, ‘관계형성’, ‘자기표현’, ‘긍정적 자기인식’, ‘자기효능감 촉진’의 음악치료 단계별 프로그램으로 구성하였다. 환자들의 기분상태 변화를 파악하기 위해 POMS 척도를 사용하여 음악치료 시행 전, 2회기, 4회기, 6회기, 8회기의 음악치료 시행 후 5번에 걸 쳐 환자들의 불안, 우울, 분노, 활력을 검사하였다. 효과 검증을 위한 자료 분석은 SPSS 12.0 을 이용하여 빈도분석(Frequency Analysis)과 다변량 분산분석(MANOVA)을 실시하였다. 총 8회에 걸친 연구결과는 음악치료가 암환자의 기분상태 변화에 유의한 효과가 있는 것으로 나 타났다. 기분상태의 하위영역인 불안과 우울은 감소하고 활력은 증가되어 음악치료가 암환자 의 기분상태에 긍정적인 효과가 있었으나, 분노에서는 유의한 차이가 없는 것으로 나타났다. 단계별로는, ‘관계형성을 위한 음악치료’와 ‘자기표현을 위한 음악치료’에서는 기분변화에 영 향을 나타내지 못하였으나 ‘긍정적 자기인식을 위한 음악치료’에서는 활력에만 유의한 효과가 나타났다. 마지막 단계인 ‘자기효능감 촉진을 위한 음악치료’에서는 분노를 제외한 불안, 우 울, 활력에서 유의한 효과가 나타났다. 따라서 음악을 통하여 여러 질환을 치료하고자 하는 노력이 증대되고 있는 가운데 음악치료가 암환자의 기분상태 변화에 긍정적인 효과를 나타낸 다면 이를 지속시키기 위한 다양한 음악치료 프로그램 구성 및 활성화를 위한 노력이 필요할 것이다.

영문초록

The purpose of this research is to investigate how the music therapy for cancer patients makes positive difference in the mood state of target, i.e., anxiety, depression, wrath and vitality. For this research, the following 31 patients were selected: 21 patients composed of 10 hospitalized patients and 11 outpatients of KC hospital located in Seoul from March 25 to May 16, 2008 and 10 inpatients from K hospice palliative care unit. Before experiment, the mood state of cancer patients, i.e., their emotional states of anxiety, depression, wrath and vitality were grasped by using POMS scale. The music therapy was performed by applying 8-time program in 2 times a week for 4 weeks. POMS scale was used in 5 times to check the mood state of cancer patients, i.e., before performance, 2nd term, 4th term, 6th term and 8th term. To verify the effect, Frequency Analysis and multivariate verification(MANOVA) were performed using SPSS 12.0. The research showed that the mood state of patients, anxiety, depression, wrath and vitality, the music therapy diminished anxiety and depression and increased vitality, showing significant effect. However, no significant difference was observed in wrath. Patients did not show any difference in the mood state at ‘music therapy to form relation’ and ‘music therapy for self expression’ stage. However, significant effect was shown only in the vitality at ‘music therapy for positive self recognition’ stage. At ‘music therapy for promoting self efficacy’ stage, the result showed significant effect in anxiety, depression and vitality except wrath. Therefore, while music is widely adopted to treat diverse diseases, as music therapy has positive effects on the mood state of cancer patients, developing various music therapy program should be encouraged to sustain and promote the effects.
