연구목적 본 연구는 인구의 고령화로 인한 영향과 건강보험 재정악화의 주요 요인분석을 분석하고, 이를 통해 현행 건강보험 지불제도인 행위별수가제의 한계와 타당성을 검토하고, 개선방안을 제시하는데 그 목적 이 있다. 연구방법 본 연구는 건강보험심사평가원 진료비통계지표, 2012년 노인실태조사, 보건복지부와 한국보건사회연구 원이 개최한 100세 시대 대비 저출산고령사회포럼 등 문헌조사를 통해 고령사회 대비한 건강보험 지 불제도 개선방안에 대해 분석하였다. 연구결과 인구의 고령화는 의료비 및 사회보험료 상승 등 개인과 국가의 부담으로 작용하여 사회보험 재정악화 를 초래하는 등 사회통합과 경제발전에 중요한 영향을 미칠 것으로 전망되고 있다. 그 이유는 노인층 연령대는 다른 인구층보다 보건의료 이용량이 많고, 현재로서는 이를 통제할 기전이 없기 때문으로 판 단된다. 결론 의료서비스의 내용도 급성기질환에서 만성질환 내지는 장기요양으로 전환될 가능성이 높다. 이는 현행 건강보험 지불제도와 부합하지 않아, 건강보험 재정의 악화를 초래할 것으로 예상된다. 결국, 인구의 고령화에 따른 의료비와 건강보험의 재정악화는 국민의 부담으로 직결되고 건강보험의 지속가능성 의 구심을 제기하게 될 것이다. 따라서, 고령사회에는 진료비를 예측 관리할 수 있는 건강보험 지불제도 로의 개선의 필요성이 대두되고 있다.
Objective : This study is the purposeis that ia analysis on affected due to the aging of the population and analysis of the main factors of the health care financial deterioration. so, This study is to suggest that improve the payment of health insurance through Review of limits and validity of the current health care payment system. Methods : This study medical expenses statistical indicators Health Insurance Review and Assessment Service, 2012 Senior Survey, Ministry of Health and Welfare and the Korea Institute for Health and Social Affairs held aging society forums and literature against the aging society and health care payment system improvement about analyzed. Results : The aging of the population is expected to have a significant impact on the social integration and economic development, such as medical expenses and social insurance premiums rise at the expense of personal and national acts that lead to the deterioration of the social insurance financing. That's why the amount of elderly ages’s health care use is many more than the other ages of that because there is currently considered that is no mechanism on control it. Conclusion : Medical services likely to be transformed into a chronic disease or long-term care from acute disease. It is not consistent with the current health care payment system that is expected to result in the deterioration of the health insurance system. In the end, the financial deterioration of the medical expenses and health insurance due to the aging of the population at the expense of the people and the sustainability of the health insurance will be raised doubts concerning. Therefore, an aging society has become a necessity that the payment of health insurance can be improvements in order to predictions or managed with the amount of medical expenses or health insurance premiums.
