본 연구는 치매주간보호소를 이용하는 치매노인에게 케어제공과 관련된 자의 인식변화를 도모하고 케어매니지먼트를 실시하는 수단으로써 ｢KOMI 차트시스템｣을 실천현장에 적용해봄으로써, 경험적 사례적용을 토대로 재가노인복지 실천현장에서의 케어매니지먼트 적용을 위한 개선방안을 제시하고자 하였다. 그리고 2008년 노인수발보험제도를 앞두고, 클라이언트의 다차원적인 욕구에 따른 서비스 전달의 효율성과 효과성을 증진시킬 수 있는 방법을 찾고자 하였다. 5개월 동안의 ｢KOMI 차트시스템｣실시에 의한 결과는 이용노인의 경우 주간보호 생활에서 정서적 안정감으로 인한 문제 행동이 감소되고 감정의 기복이 줄어 돌발적인 행동이 줄어들었다. 특히, 표현력 증가 및 감정표현의 폭이 넓어짐으로 인해 과거에 익숙했던 잔존능력을 활용함으로써, 생활의 활력이 증가됨을 발견할 수 있었다. 또한 치매어르신에게 있어서는 이용시설의 환경과 생활하는 주거환경이 매우 중요하다는 인식을 가질 수 있게 되어 “내 집 같이 편안한 주간보호실”이라는 테마로 실내 환경을 바꾸기에 이르렀다. 특히 일생을 거쳐 생활하는 집에서 많은 비중을 차지하는 부엌가구를 설치함으로써 이용 노인들의 생활에 많은 변화가 발생하는 것을 확인 할 수 있었다. 이는 ｢KOMI 차트시스템｣이 추구하는 ‘케어의 다섯 가지 척도’는 케어를 실행함에 있어 케어의 방향성을 제시함으로써 그 케어의 지표가 되었으며, 케어자 중심에서가 아닌 이용자 중심의 케어방향으로 인식이 변화한 것에서 비롯되었다고 추측된다. 결론적으로 KOMI 이론을 바탕으로 만들어진 KOMI 차트시스템의 사용은 케어제공자의 인식 및 케어의 질 향상, 그리고 치매노인의 생활의 질 향상을 가능하게 할 수 있는 케어매니지먼트 도구 중의 하나라는 가능성을 확인할 수 있었다.
The purpose of this study is to apply KOMI chart system according to care management at day care center for Dementia, aiming at perception change of carer. And also, applied KOMI chart system as care management has shown effective strategies for Dementia at in-home care field. And this study aim at efficiency and effectiveness of service deliverly according to multidimentional needs provide for the 2008 long-term care system. The result of KOMI chart system study for five months are affirmative aspects as follows: First, user at day care center for dementia senile decreased in trouble and sudden behavior because of emotional stable. Especially, it showed increase of vitality as making use of survival abilities. Second, utility facilities and life environment for dementia senile is found very important and tried to alter a part of the interior of day care center for dementia on the subject of homelike day care center. With equipping kitchen appliances, user at day care center for dementia senile enriched their life. The grounds of this outcome is arose from a different angle, that is not carer but user support the dementia senile. And the five scale of care in KOMI chart system in term of directing of care proved the index of care. In conclusion, KOMI chart system based on KOMI theory could be confirmed the tool among care management tools to evaluating carer preception, care quality and life quality for dementia senile.
