목적 : 본 연구는 작업치료사가 근무하는 전국 의료기관의 전자의무기록시스템(Electrical Medical Record; EMR) 도입 수준 및 임상 용어 사용 현황을 알아보고, 향후 작업치료 임상용어체계의 표준화를 위한 기 초자료로 활용하기 위하여 실시하였다. 연구방법 : 전국의 작업치료사들이 근무하는 121곳의 의료기관을 대상으로 작업치료 서비스에 대한 임상 용어 사용 현황을 설문조사하였다. EMR을 사용하지 않는 경우와 사용하는 경우로 나누어 분석하였으며, SPSS의 다중응답(multiple response) 모듈을 이용하여 빈도분석을 적용하였다. 결과 : 조사대상 총 121개 기관 중 EMR을 사용하는 기관은 102개, 84.3% 였다. 치료예문이 미리 입력 되어 있고 시행 내용을 불러오는 형식으로 예문 수정이 가능한 경우가 많았으며, 사용하는 작업치료 용 어가 수행중인 치료 영역을 포함하는가에 대한 질문에서 60.0%이상이 부족하다고 응답하였다. 작업치료 임상용어의 표준화 필요성에 대해서는 85.1%가 필요하다고 응답하였으며, 협회에서 공식적인 임상용어 표준안을 제시할 경우 74.5% 기관에서 적용의사가 있다고 응답하였다. 결론 : 본 연구를 바탕으로 국내 작업치료사의 직무와 국제의료행위분류, Current Procedural Terminology (CPT) 코드, Occupational Therapy Practice Framework (OTPF) 용어 등에 근거한 한국화된 작업치 료 임상용어 표준화 작업이 이루어져야 할 것이다.
Objective : This study was conducted to be used as an essential resource for standardizing a future medical terminology framework by analyzing the current status of the Electronic Medical Record (EMR) system and medical terminology used in health-care institutes where occupational therapists are employed. Methods : A total of 121 health-care institutes where occupational therapists are employed completed surveys regarding the current status of occupational therapy related to medical terminology use. This study was divided into two categories performed individually based on the use of the EMR system. The data were analyzed using an SPSS multiple response module. Results : Among the 121 health-care institutions, 102 institutions use the EMR system, which is a rate of 84.3%. Treatment plans are already recorded in the system, and the progress notes can be edited by pulling out the recorded data. More than 60.0% of the institutions expressed limitations in using the occupational therapy treatment terminology of the system. In addition, 85.1% indicated the necessity of standardizing medical terminology, and 74.5% showed a strong willingness to apply standardized terminology if the Korea Association of Occupational Therapists (KAOT) approves an official standardized medical terminology framework. Conclusion : The standardization of Korean occupational therapy medical terminology needs to be promoted by incorporating the International Classification of Health Intervention (ICHI), Current Procedural Terminology (CPT) code, and Occupational Therapy Practice Framework (OTPF) terminology.
