목적 : 본 연구는 정신장애인을 대상으로 작업치료 영역에서 직업재활에 대해 알아보기 위해 진단군 및 중 재의 유형과 효과에 대하여 국외 연구들을 중심으로 체계적 고찰을 실시하고자 한다. 연구방법 : 2010~2019년까지 자료 검색을 위해 PubMed, EMBASE, OTseeker 둥 총 12개의 데이터베 이스에서 Mental disorder, Mental illness, Psychiatric disorder, Psychotic disorders, Bipolar disorder, Depressive disorder, Schizophrenia, Schizoaffective disorder, Employment, Job, Work, Vocational, Occupational therapy를 검색하였다. 최종 분석에 포함된 연구는 총 29편이었다. 결과 : 연구에 포함된 진단군은 조현병이 가장 많았다. 직업재활을 중재 유형별로 살펴보면 지원고용이 16 편로 가장 많았고, 사회기술 및 인지재활 훈련+지원고용 10편, 사회기술 및 인지재활 훈련, 기타 직업적 접근법, 사회기술 및 인지재활 훈련+기타 직업적 접근법은 각각 1편이었다. 고용 유형에 따른 중재 유 형별 고용률에서 효과를 나타낸 연구를 살펴보면, 경쟁고용에서는 지원고용이 13편, 비경쟁 고용에서는 기타 직업적 접근법이 1편, 유급고용에서는 지원고용이 3편으로 각각 가장 많았다. 결론 : 추후 국내 정신장애인을 대상으로 체계적이고 종합적인 직업재활서비스를 제공하기 위한 기초자료 와 정책과 평가체계 도입의 근거로 활용할 수 있을 것이다.
Objection: In this study, a systematic review was carried out to overseas studies and determine diagnostic groups and the types, effects of interventions, in order to understand vocational rehabilitation in the area of occupational therapy for people with mental disorders. Methods: For data retrieval from 2010 to 2019, a total of 12 databases, including PubMed, EMBASE, and OTseeker, were searched for Mental disorder, Mental illness, Psychiatric disorder, Psychotic disorders, Bipolar disorder, Depressive disorder, Schizophrenia, Schizoaffective disorder, Employment, Job, Work, Vocational, Occupational therapy. A total of 29 studies were included in the final analysis. Results: The diagnostic group included in the study had the most schizophrenia. Looking into vocational rehabilitation for each of the types of interventions, 16 papers, the largest, were about supported employment, followed by 10 about social skills and cognitive rehabilitation training plus supported employment and one for each regarding social skills and cognitive rehabilitation training, other vocational approaches, and social skills and cognitive rehabilitation training plus other occupational approaches. Regarding studies showing effects in the employment rate by interventions type following types of employment, as the largest number of studies, 13 papers were about supported employment in competitive employment, one were about other vocational approaches in non-competitive employment and three about supported employment in paid employment. Conclusions: In the future, It can be used as a basis for introducing basic data, policy and evaluation system to provide systematic and comprehensive vocational rehabilitation services for people with mental disorders in Korea.
