정신보건 영역에서의 사례관리는 전통적으로 만성정신장애인을 중심으로 진행되어 왔으나, 최근에는 다양한 정신건강 상의 문제나 욕구를 가진 대상층,즉 아동 및 청소년 연령층,우울 장애를 가진 집단,자살생존자나 자살고위험군에 대한 사례관리 등 정신건강상의 다양한 문제 영역으로 그 대상범위가 확대되고 있다.또한 사회구조의 변화에 따른 사각지대 해소 차원에 서 최근 지역사회서비스투자사업(CSI)중에서 CSI정신보건서비스가 일부 시행되고 있는 상황 이다.본 논고는 최근의 정신보건서비스 환경의 다양한 변화들을 전통적인 정신보건서비스 영 역과 사회서비스인 CSI영역으로 나누어 정신보건사회복지 영역의 서비스 실천현황을 사례관 리 실천을 중심으로 살펴보고 앞으로의 과제들을 다음과 같이 탐색적으로 고찰하였다.첫째, 전통적인 치료와 재활의 관점이 아닌 정신건강에 대한 예방 및 조기개입으로의 패러다임 전 환이 필요하다.둘째,재활서비스 제공을 인적자본의 개발을 위해 근로자지원프로그램(EAP)의 관점이 필요하다.셋째,기존의 국가가 공급자인 형태에서 공공과 민간의 정신보건 전달체계 의 합리를 위해 민간의 자율성을 보장하는 방식으로 정신보건체계의 공공성과 간격을 좁히려 는 노력이 필요하다.마지막으로 향후 정신보건서비스는 지자체에서 그 지역사회의 정신보건 욕구를 파악하여 지역특성에 맞는 정신보건서비스 프로그램을 개발해나가도록 유도해야 할 것이다.
Case management in the area of mental health has traditionally been made centering around persons with chronic mental disability, but in recent days, its range has been expanded to groups with various problems or needs in mental health, that is to say various aspects of mental health including children, youth, groups with depressive disorder and case management on suicide survivors or suicide high risk groups. Also, a part of CSI mental health services among Community Social Service Investment(CSI) has recently been implemented to resolve blind spot according to changes in social structure. This study looked into the present condition of practice of case management in the area of mental health social welfare by dividing various recent changes in the environment of mental health services into traditional mental health services and CSI area which is a social service, and considered future assignments as follows. First, change of paradigm to prevention and early intervention in respect of mental health is required, not the perspective of traditional treatment and rehabilitation. Second, perspective to view provision of rehabilitation service from Employee Assistance Program(EAP) for the development of human capital is necessary. Third, breaking from existing role as a supplier, the nation must take effort to secure publicness of mental health system and narrow the space through the method of guaranteeing private sector’s autonomy for rationalization of delivery system for mental health of both public and private sectors. Lastly, with respect to future mental health services, local governments must take effort to induce development of mental health service programs matching local communities by understanding community’s specific needs for mental health.
