통증은 일상생활을 어렵게 하는 증상으로 의료기관을 찾게 되는 주요한 원인이다. 노인이 경험하는 통증은 만성질환 또 는 노화와 관련된 증상으로 일시적인 해결책 보다는 장기적인 관점에서 보아야 한다(American Geriatrics Society, 2002). 2011년 전국노인실태조사에서 우리나라 노인의 만성질환은 고혈압, 골관절염, 당뇨병, 요통 및 좌골신경통, 고지혈증 순 으로유병율이높았다. 노인의88.5%가만성질환이있었으며 평균 2.5개의 만성질환을 앓고 있었다(Chung et al., 2012). 70세 이상 남성의 25%, 여성의 51%에서 신체의 두 곳 이상 부위에 통증이 있는 것으로 보고되었다(Bergh et al., 2003). 남녀별통증부위는여성은하부요통, 어깨, 머리, 손, 무릎의 순으로통증이많았고, 남성은하부요통,무릎,머리,복부,어 깨 순이었다(Bergh et al., 2003). 만성질환노인의 상당수가 통증과 함께 우울증상을 동반하 며 통증과 우울은 정상관관계가 있는 것으로 선행연구에서 밝 히고 있다(Chang, Sohn, & Cha, 2006; Kim & Kim, 1998). 만성통증이 있는 대상자는 대개 우울을 경험하며 통증이 심할 수록 우울이 심하고, 우울이 심할수록 통증은 심하여 통증이 우울에 영향을 미치는 효과가 큰 변인임이 보고되고 있다 (Chang et al., 2006; Kim & Kim, 1998). 또한 통증과우울 은 공통적으로 기억기능저하, 일상 신체활동 저하, 수면장애 와 같은 생리적 증상과도 관련이 있다(Bergh et al., 2003; Chang et al., 2006; Miro?, Martinez, Sa?nchez, Prados, & Medina, 2011).

영문초록

Purpose: The present study was done to examine the role of depression as a mediator of the effect of pain severity on memory function, physical function and sleep disturbance in older adults with chronic disease. Methods: The participants were 158 older adults with chronic disease who were living at home. All participants completed structured symptom questionnaires. Data were analysed with the SPSS program, using descriptive statistics and regression analysis. Results: Mediation analysis suggested that pain severity influences sleep disturbance directly as well as indirectly by its effect on depression. The ratio for sleep disturbance affected by pain severity directly was 72.7% and for sleep disturbance affected by pain severity via depression indirectly, 27.3%. Conclusion: These results indicate that effect of pain severity on sleep disturbance is mediated by depression. Pain severity affected sleep disturbance directly as well. These findings suggest clinical intervention opportunities that include strategies to improve depression for better pain control which in turn will result in reduced sleep disturbance.
