최근 인구의 고령화, 만성 질환의 증가 및 복합적인 질환의 특성으로 인해 중증도가 높은 환자들이 증가하는 추세이나 중환자실 병상의 제한으로 대부분의 환자들이 일반병동에 입원하여 치료를 받게 된다. 따라서 입원기간 동안 악화 상태를 보여 심정지, 예기치 못한 중환자실 입실, 사망 등과 같은 치명적인 사건들을 종종 경험하게 된다(Devita et al., 2006). 국내 ? 외 통계청의 보고에 따르면 10대 사망원인 중 7가지가 만성질환이며(Heron, 2013; Korea National Statistical Office, 2012), 만성질환을 가진 환자의 75% 이상이 심장질환, 종양, 만성 호흡기 질환, 당뇨, 신부전, 폐렴 등의 내과 질환으로 인해 사망한다고 하였다(Heron, 2013). 국내의 한 연구에 따르면 일반병동 내 내과 환자가 외과 환자와 비교하여 비계획적인 중환자실 전동률이 4.12배 높다고 하였다. 이는 내과에 입원한 환자들의 대부분이 만성질환을 가지고 있으며, 한 개 이상의 복합적인 기저질환을 가지고 있는 경우가 많고 임상양상 또한 다양하여 악화되는 상태를 조기에 발견하기 어렵기 때문이다(Park, Hong, Lim, & Koh, 2010).

영문초록

Purpose: The purpose of this study was to analyze risk factors in predicting medical patients transferred to Intensive Care Unit (ICU) on the general ward. Methods: We reviewed retrospectively clinical data of 120 medical patients on the general ward and a Modified Early Warning Score (MEWS) between ICU group and general ward group. Data were analyzed with multivariate logistic regression and the area under the receiver operating characteristic curves using SPSS/WIN 18.0 program. Results: Fifty-two ICU patients and 68 general ward patients were included. In multivariate logistic regression, the MEWSs (Odds Ratio [OR], 1.91; 95% confidence interval [CI], 1.32-2.76), sequential organ failure assessment score (OR, 1.28; 95% CI, 1.10-1.72), PaO2/FiO2 ratio (OR, 0.98; 95% CI, 0.98-0.99), and saturation (OR, 0.93; 95% CI, 0.88-0.99) were predictive of ICU transfer. The sensitivity and the specificity of the MEWSs used with a cut-off value of six were 80.8% and 70.6% respectively for ICU transfer. Conclusion: These findings suggest that early prediction and treatment of patients with high risk of ICU transfer may improve the prognosis of patients.
